Repeat Prescriptions

Once you are stable on your medication and have progressed to tri-monthly consultations, you are eligible to apply for repeat prescriptions.

Provided that you have attended a consultation in the previous 3 months, you can use the form below to request your prescription.

Should you need to change your prescription, or if it has been over three months since your last consultation, you will need to book an appointment with your consultant to discuss your needs.

Please note that personal data submitted using this form will be used for the sole purpose of processing your repeat prescription.

• Please note that repeat prescription requests may take up to 24 hours for approval, after which an invoice will be sent to your email. To ensure timely processing, submit your repeat prescription requests by 12:00 PM on Friday and complete payments by 4:30 PM on the same day. Requests and payments made after the deadlines will be processed on the next working day.
